| The
requirements of this document are mandatory and Greencore Group shall be expected
to demonstrate compliance with relevant sections “So far as is Reasonably
Practicable”. This
document is not for general distribution without prior discussion with Corporate
Office Runcorn. This
document aims to clearly identify the requirements for accident and near miss
reporting within the Greencore Group. There
are duties under some existing health and safety legislation that require employers
to conduct an accident investigation and take account of lessons learnt. We accept
that preventing absences from work caused by occupational injuries or ill health
is an important part of every manager’s responsibilities. Unless the causes of
workplace accidents and ill health are properly understood, lessons will not be
learned and suitable improvements will not be made to secure the future health
and safety protection of those who may be affected by our work activities. The
prime responsibility for accident and ill health investigation and prevention
within Greencore Group lies with the “Duty Holder e.g. Site Director or Manager
for that business. For
every accident that takes place there is a large number of “Near Misses” or events,
which have the potential to cause harm. It is the “Duty Holders” responsibility
in consultation with employees to document the level of “Near Miss Reporting appropriate
for each operating location. As
a major European employer we regularly undertake accident and disease investigation
when their has been an event in the workplace which has caused injury or ill health
in order to ensure lessons are learnt but investigative practices vary across
the business. This policy has been developed to introduce a degree of uniformity
in the information we gather, why and what we effectively utilize that information
for. The
Reporting of Injuries, Diseases and Dangerous Occurrences Regs (RIDDOR). Employers
have to keep records of injuries at work and report certain types to the appropriate
enforcing authority usually the Health and safety Executive (HSE) or a local authority
e.g. Environmental Health. Reportable injuries include fatal and major injuries
to employees, self employed people, and members of the public, and injuries that
cause incapacity for work for more than three days including Bank Holidays and
weekends (over 3-day injuries). We do not propose to elaborate upon statutory
provision. The reader must refer to the reporting requirements contained in RIDDOR,
which must be consulted in conjunction with this policy document. The
primary role of the accident investigator is to identify the root cause of the
accident or ill health and thus assist in preventing a reoccurrence. It is not
the role of the investigator to apportion blame, to admit or decline liability
This is the responsibility of the “Duty Holder” (see above). All
documentation remains the property of Greencore Group and is therefore subject
to the rules of privilege. All requests for the release of documentation that
is not covered by statutory provision examples of which would be, RIDDOR or (Reg
3) the Management of Health and Safety at Work Regulations etc must be via the
relevant company insurer. What
to do in the event of an accident or occupational illness? ... within 24 Hours.
At the Location of the accident. Ensure
any injured personnel receive first aid and prompt hospital treatment as required.
All minor injuries should go to hospital accompanied in a company car or taxi.
Ambulances should be called for all serious incidents. - Inspect
all equipment and working areas for defects keep a record as appropriate.
- Quarantine
defective tools and equipment.
- Keep
a record of any obstacles in the area.

Keep a record of any foreign substances in the area.
Documentation
- Complete
the accident book (DSS BI510).
- Complete
any RIDDOR documentation e.g. F2508 & F2508A etc.
- Complete
the company accident investigation report.
- The
incident (however minor) must be entered on the on ARRAN3 accident database.
Photographs
- Photograph
the scene of the accident it can save a thousand words in a report.
Remember
digital images may not be admissible in court. Obtain
written statements as a matter of priority. - The
Supervisor or line manager ASAP.
- The
injured person subject to receiving treatment.
- All
eye witnesses.
- Other
employees (personnel) in the area even if the did not witness the incident
e.g. negative statements. Note. obtain adequate contact details of
all non-Greencore Group personnel, particularly delivery drivers etc. The
Injured employee (individual) etc. Record - The
task being carried out at the time of the accident.
- Whether
the task was part of the persons normal job.
- Was
the person authorized to perform the task?
- Was
the person authorized to be in the work area and if not why were they there?
- Any
breaches laid down from the approved “Safe System of Work”
- Who
was the first person on the scene?
- Was
the accident reported and so by whom?
- If
the accident was not reported why not?
- How
long after the incident was the accident reported?
- Record
any changes to the scene of the accident,
which have occurred subsequent to
the accident. Within
48 Hours after the accident you must consider. If
the injured person is off work, it is important that you make visual contact
with them. Ideally ask them to call into work. If this is not possible,
make an appointment to call upon them at home or visit them in hospital.
- Record
any actions taken.
- Record
any additional factors relating to the injured and or the accident.
Reviewing
risk assessments and ‘Safe Systems of Work’. Record
any subsequent changes to, - The

- Laid
down operating procedures.
- Training
and education.
- The
scene of the accident or access.
- Any
machinery involved.
- Make
specific note if any of the changes were unrelated
to the circumstances of
the accident. All
accidents, dangerous occurrences, and occupational diseases Involving any employee,
or visitor arising out of the activities carried on or within company premises,
must be reported to the Head of Department and recorded in an accident book Form,
BI 510 must be used. The
Health and Safety Advisor will enter the details contained on the accident forms
onto a computer database linked to a Corporate Office. Statistical data will then
be produced at the end of each financial year, which will be made available to
management, Insurers and other interested parties on request. The
Corporate Health and Safety Advisor will investigate accidents where it is considered
that avoidable hazards have been a contributory factor, or where there may be
implications for other factories. All reasonable efforts will be made to prevent
a reoccurrence of such accidents. The

fatalities
- Fractures
(including breaks, cracks or chips) of skull, spine, pelvis, hand, or leg
- Amputations
- Any
electric shock
- Loss
of sight to an eye,
penetrating wound to the eye, or chemical/metal burn
to the eye - Loss
of consciousness resulting from lack of oxygen
- Any
illness/loss of consciousness from substance by inhalation,
ingestion or absorption
through the skin - Acute
Illness caused by exposure to pathogen or Infected material
- Any
injury requiring hospitalisation by ambulance
or personnel being detained
